What to Do First

When a death occurs, the first step is to contact us at any time of day or night. We are available 24 hours a day, 7 days a week to assist you and provide immediate guidance.

If Death Occurs at Home

If the death occurs at home, you should contact the following in this order:

• If your loved one was under hospice care, call the hospice nurse
• If your loved one was not under hospice care, call 911
• Call us at 718-456-2800

If Death Occurs at a Hospital/Nursing Home

If the death occurs at a hospital or nursing home:

• The medical staff will contact the family and the proper authorities
• Contact us at 718-456-2800
• We will coordinate with the medical facility

Information We'll Need

When you contact us, we'll ask for:

• Your name, address and phone number
• Name of the deceased
• Location of the deceased
• Name of the hospital/nursing home (if applicable)
